Elderly Stroke Rehabilitation : Incidence and Risk Factors for Medical Complications

Abstract
Objective: Medical complication during stroke rehabilitation is a common occurrence. There has been not much studies dealing with medical complications occur during rehabilitation of elderly stoke patients. This study aims to identify the incidence and risk factor of medical complications during rehabilitation in elderly stroke patients.

Methods: Stroke patients admitted for inpatient between July 2007 and June 2012 were retrospectively evaluated throughout rehabilitation to identify incidence of complications. Demographic and stroke information impairment, preexisting medical conditions, and laboratory abnormalities were recorded.

Results: Eighty-one percent of patients experienced at least one complication. The most common complications were dysphagia, shoulder pain, post-stroke depression, and pneumonia. The incidence of complication was significantly higher in patient who had Brunnstrom stage ¡Â 2, foley catheter insertion, tracheostomy insertion, and high score of National Institutes of Health Stroke Scale (NIHSS). The patients with complication had longer period (30.64 ¡¾ 26.65) from onset to transfer rehabilitation unit than the patients without complication (17.91 ¡¾ 10.52). The patients with complications had longer rehabilitation hospital stay (p<0.001). Brunnstrom stage of arm ¡Â2 (odds ratio [OR]=8.25), NIHSS ¡Ã15 (OR=5.75) was statistically associated with stoke-related complications.

Conclusion: Medical complications are common among patient undergoing stroke rehabilitation in old age. The risk factors of medical complications are Brunnstrom stage of arm, hand, and leg ¡Â 2, foley catheter insertion, tracheostomy insertion, and high score of NIHSS.
